Game Report Process

This form  submits:
1) game details for each game played
2) a photo of the reporting  team’s scoresheet.

Confirmation of submission:
After submitting the form, you should receive a “Thank you message advising your request has been submitted”.
A copy of your submission is sent to the email address provided.  Please review your submitted data for input errors.

If you do not receive one of these confirmations, please resubmit and escalate to your division convener.

Beware: We are aware of one problem with submission from a mobile device!  Client was able to submit using his personal computer.
